Sheffield SENDIAS
  • SSENDIAS has introduced a new referral form to help us understand the needs of parents, carers, children and young people and ensure that the most appropriate level of support is provided.

    When you complete the form, we will ask you to briefly tell us:

    • what you would like help with;

    • what has happened so far;

    • how the situation is affecting you or your child or young person;

    • what you would like to happen next; and

    • about any important meetings, decisions or deadlines.

    What happens after I submit the form?

    Your referral will be reviewed by the SSENDIAS team. We will consider the information you have provided and identify the most appropriate way to support you.

    SSENDIAS provides different levels of support depending on individual needs and circumstances. This may include:

    Parent/carer/young people information sessions
    Providing information about SEND rights, processes and common issues so that you feel more confident about your options and next steps. (put on link to these)

    Information and advice
    An Information Officer may speak with you about your concerns, explain your rights and options and help you identify practical next steps.

    Individual casework support
    Where more individual or ongoing support is needed, your referral may be allocated to a Caseworker.

    Support for young people
    Where appropriate, our Young People’s Involvement Officer can provide direct support to help young people understand their rights and choices and make sure their views are heard.

    Completing a referral does not automatically mean that you will be allocated a Caseworker or Young Peoples Involvement Officer. Our aim is to match you with the right level of support for your needs and help you build the knowledge and confidence to take things forward wherever possible.

    SSENDIAS continues to provide a free, impartial and confidential service to parents, carers, children and young people.

    This referral form is new, and we want to make sure it works well for the people using it. If you found anything confusing, difficult or felt something was missing, please let us know. We will use your feedback to help make the form simpler and more accessible.

  • We are currently experiencing a high number of referrals which means things may look a little different. Please click here to read our full busy period statement
